National Healthcare Group eyes digital records for community care

Community care partners of the National Healthcare Group in Singapore have been provided access to a digital platform for managing health programmes and potentially creating digital records.

WHAT IT’S ABOUT

Recently, the NHG partnered with credentials platform provider, Accredify, to develop the NHG Cares Partners Portal.

According to a press statement, the portal simplifies the management of health programmes, which are automatically synced with NHG’s health and social care marketplace, Health Kampung. It allows NHG community partners to monitor demand by tracking registration and attendance and real-time updates.

Through the portal, NHG partners can also access and leverage insights from anonymised population health data to design tailored community-based programmes.

Additionally, NHG partners can issue e-certificates to their participants to recognise their efforts in managing their health and fitness, further encouraging self-health management in the community.

Later, NHG residents could access shared care services via the portal, including screening services and social prescribing.

The portal, which now has more than 20 community partners onboarded, has already automated over 400 health programmes since its launch in April.

WHY IT MATTERS

NHG empowers those in the community to take more control of their health by bringing community care resources close to them.

“The [p]ortal empowers our community partners to effectively reach out to residents and activate them towards better health and well-being with relevant programmes,” said Ong Jing Fang, chief of Population Health at NHG.

Accredify also shared that the organisation can potentially introduce verifiable healthcare records through the online portal. “The NHG Cares Partners Portal creates the necessary infrastructure for enhanced care continuity and personal ownership over health information,” said CEO and co-founder Quah Zheng Wei.

THE LARGER TREND

The NHG has also implemented the Next Generation EMR (NGEMR) system as part of its digital transformation. The system progressively went live across the cluster, starting with the National Healthcare Group Polyclinics in 2021. Tan Tock Seng Hospital then rolled out the system the following year, becoming the first hospital in the cluster to go live with the integrated EMR system. Developed by Synapxe, the NGEMR is also live across the National University Hospital System cluster.
